Work through the sections that match your claim type. Tick items you already have, note gaps, and ask Claims Saathi or your insurer/TPA which items apply to your matter.
Before you begin
- Keep copies of everything you submit
- Prefer clear scans or photographs with readable dates and amounts
- Do not share passwords, OTPs or full card details on public forms
- Use secure channels when identity or medical documents are requested
Documents commonly requested across claim types
Documents commonly requested may include identity and bank details for settlement, policy references and claim forms. Your insurer or TPA will confirm what is mandatory.
General checklist
- Completed claim form (as prescribed by insurer/TPA)
- Policy details or policy schedule reference
- Identity proof of claimant / insured (as required)
- Bank account details for reimbursement settlements (via secure channel)
- Authorisation letter if a representative is coordinating
- Any written query responses previously requested
Health claim documents
Often relevant for hospitalisation claims
- Hospital discharge summary
- Detailed hospital bill with break-up
- Payment receipts / settlement proof where applicable
- Investigation and pharmacy bills
- Doctor’s prescriptions and investigation reports
- Pre-authorisation approval letter for cashless cases (if issued)
- Implant stickers or invoices when relevant

Motor claim documents
Often relevant after an accident or damage
- Claim intimation reference
- Driving licence and vehicle RC copies (as required)
- FIR or police report where applicable
- Estimate / repair quotation
- Photographs of damage
- Survey / inspection report references when available
- Invoices and payment proofs for approved repairs
Common mistakes
- Submitting incomplete bill break-ups
- Unreadable scans of key pages
- Mismatch between names on bills and policy
- Missing discharge summary pages
